I encourage staff to report even the smallest change in a resident’s condition. From orientation forward, I stress the importance of noticing subtle differences and communicating them right away. Early recognition gives us valuable time to intervene before concerns become critical.

When team members share what they see, we respond. We assess the situation, determine next steps, and document findings thoroughly. This approach reinforces that every observation matters.

Daily Oversight and Timely Intervention

Stand-up meetings and rounding strengthen our early detection efforts. Through direct care, staff observe residents receiving services every day. In addition, we intentionally lay eyes on every resident daily, regardless of care level. This consistent presence allows us to recognize changes quickly and respond appropriately.

Early identification also expands our options. We can collaborate with the PCP, initiate therapy, involve home health, or introduce hospice when appropriate.
